Required Forms for Players

Medical Release Form

The Medical Release Form is REQUIRED for every player. Please complete this form for each child. A parent or guardian must electronically sign the form. After signing, a copy will be emailed to you and to Scarborough Little League.

School Enrollment Verification Form

This form is NOT required to complete Little League registration. However, it may be requested by Scarborough Little League to verify eligibility per new Little League enrollment rules. If needed, you will be contacted directly.

If the form is required, a school official must complete it. It may be emailed to registration@scarboroughlittleleague.org. A signed hard copy may also be requested.

All Star Verification Form

This form is required only for Tournament players participating in the Minors/10U, Majors/11U, or Majors/12U post-season tournaments. Scarborough Little League will provide instructions after All-Star rosters are announced. Questions may be directed to registration@scarboroughlittleleague.org.
