The Medical Release Form is REQUIRED for every player.  Please completely fill this form out for each player and a parent or guardian must electronically sign this form.  After signing, it can be emailed to [email protected]:

The School Enrollment Verification Form is NOT required to complete Little League registration for your player.  However, due to new Little League enrollment rules it may be required for your player to verify suitability for enrollment in Scarborough Little League.  Scarborough Little League will contact you directly if it must be completed.  If a parent is required to have this form completed by a school official, a copy of the completed may be emailed to [email protected] and we may require a signed, hard copy as well:

The All Star Verification Form is required only for Tournament players to enter the Minors/10U, Majors/11U, and Majors/12U post-season Little League tournaments.  More instructions for completion of this form will be provided by Scarborough Little League following the selection and announcement of post-season Tournament rosters.  Questions may be directed to [email protected].
